Documentation

In order to complete the employment process at Penn State Harrisburg, you will need to fill out paperwork required by the Payroll Department at University Park.

Please come to the library prepared to provide the necessary documentation for the following forms (the actual forms will be supplied by the library):

1) Employment Eligibility Verification (USCIS Form I-9)

To complete Form I-9, bring ONE of the following:

  • Driver's license
  • Penn State Photo ID card

You must also bring ONE of the following for Form I-9:

  • Social Security card
  • Original or certified copy of your birth certificate
  • Certification of Birth Abroad issued by the Department of State (Form FS-545 or Form DS-1350)
  • U.S. Citizen ID card (INS Form I-197)

2) Employee's Withholding Allowance Certificate (Form W-4)

3) Salary Deposit Request

Payroll checks are no longer issued to students. You must accept payment as direct deposit into a checking or savings account.

  • Checking account: Bring a voided check.
  • Savings account: Bring your savings account number and bank routing number (these numbers are on your deposit slip).

International students will also need to complete the following two forms:

4) Request for Taxpayer Identification Number and Certification (Form W-9)

5) Alien Information Request Form

Please Note:

Until this required paperwork is completed and forwarded to the University Park Payroll Department, you will not be able to start work.
